National Healthcare Decisions Day exists to inspire, educate and empower the public and providers about the importance of advance care planning.

Throughout life you have many choices, including those choices about how you want the end of your life to be. It is important to think about your options in advance and share your views with your loved ones.

Then, when the time comes to make decisions, if you can’t decide for yourself, your loved ones will know that they are doing what you would have wanted. Making these decisions ahead of time will help your care providers know how to take care of you. It’s difficult to have these important discussions with your family and friends, but if we can help you plan, we are hoping that fewer families and health care providers will have to make the difficult decisions.

The ultimate goal is to honor your wishes and the first step is to understand what your wishes are.

Advance care planning is a process of understanding, reflecting on and discussing future medical decisions, including end-of-life preferences.
It may include:
- Understanding your health care condition and treatment options.
- Clarifying your health care wishes and defining what living well means to you.
- Weighing your options about what types of care and treatments you would want or not want.
- Communicating your wishes and sharing any documents with your family, friends, clergy, advisors, physicians and other health care professionals.
- Choosing a health care agent who can speak for you if you become injured or ill and unable to communicate, and deciding to complete an advance directive
